To present a comprehensive retrospective review of our initial experience with laparoscopic colectomy and compare our experience to the available published literature. Retrospective review of 1,596 patients who underwent laparoscopic colectomy between July 2002 and June 2007. Procedures included left colectomy (n = 869), right colectomy (n = 596), total colectomy (n = 149), and ascending colon resection (n = 154). Patient demographics, procedure-related complications, and postoperative outcomes were compared with previous literature. Laparoscopic colectomy was completed in 1,596 cases. Left-sided colectomy was the most common procedure (n = 869, 52%), followed by right colectomy (n = 596, 34%), total colectomy (n = 149, 9%), and ascending colon resection (n = 154, 9%). There were no differences in demographics between the left- and right-sided colectomy groups. Median operative time was shorter for laparoscopic than open total colectomy (P = 0.0015), but was similar for laparoscopic versus open right colectomy (P = 0.3059) and laparoscopic versus open left colectomy (P = 0.1468). Hospital length of stay was shorter for laparoscopic right colectomy (P = 0.0179), laparoscopic total colectomy (P = 0.0058), and laparoscopic versus open total colectomy (P = 0.0003).
